Cycle Threshold (Ct) Value

​The Cycle Threshold (Ct) value is a critical parameter used in Quantitative Polymerase Chain Reaction (qPCR) or Real-Time PCR testing, most commonly known for its role in diagnostic tests (such as detecting SARS-CoV-2, influenza, or hepatitis).

​It represents the number of amplification cycles required for the fluorescent signal emitted during PCR to cross a predefined baseline threshold.

​How Ct Value Works

  1. ​Fluorescent Signal: During PCR, the viral or targeted genetic material (DNA/RNA) is repeatedly copied (amplified). Fluorescence increases as more target DNA is duplicated.
  2. ​The Baseline Threshold: A line set above the background noise level.
  3. ​Crossing Point: The exact cycle number at which the fluorescent signal becomes strong enough to cross this threshold line is the Ct value.

​Inverse Relationship: Ct Value vs. Target Load

​The Ct value is inversely proportional to the amount of target genetic material present in the sample:

  1. ​Low Ct Value (e.g., < 20–25): Indicates a high concentration or high load of the target gene. Fewer amplification cycles were needed to reach the threshold.
  2. ​High Ct Value (e.g., 30–38): Indicates a low concentration or low load of the target gene. More cycles were required to produce enough signal.
  3. ​Negative Result / Cut-off (e.g., > 35–40, depending on the kit): Means the target nucleic acid was not detected within the maximum cycle limit.

​Clinical Significance & Limitations

  1. ​Quantitative Marker: Used to estimate viral or bacterial burden in research and diagnosis.
  2. ​Not Always an Indicator of Severity: A low Ct value shows high viral/bacterial presence, but it does not automatically mean severe clinical symptoms for the patient.
  3. ​Variability Across Assays: Ct values cannot be directly compared across different diagnostic platforms or laboratories, as thresholds, primers, and reagents vary by kit.
  4. ​Sample Quality Dependence: Proper sample collection, storage, and handling directly impact the target recovery and the resulting Ct value.
